Comparison of depression, loneliness, quality of life and cognitive levels in elderly people living in a nursing home, living at home with their families or living alone

Abstract

Aim: Our study aimed to compare the depression, loneliness, quality of life, and cognitive levels of the elderly living alone, in a nursing home, or with family members.

Material and Methods: 120 people aged 65 and over were included in the study. “Geriatric Depression Scale (GDS)” for depression levels, “University of California, Los Angeles Loneliness Scale (UCLA-LS)” for loneliness levels, “Mini Mental State Examination (MMSE)” and “Clock Drawing Test (CDT)” for cognitive levels, and “World Health Organization Quality of Life Module for the Elderly (WHOQOL)” for quality of life were used. SPSS 27.0 program was used in the analysis of the data and the significance level was determined as α=0.05.

Results: The GDS score was found to be significantly (p<0.05) higher in the nursing home and living alone groups than in the group living with family members at home. In the group living alone, the UCLA-LS and CDT score were found to be significantly higher (p<0.05) than the other groups. In the group living at home with family members, the WHOQOL and MMSE scores were found to be significantly higher (p<0.05) than the other groups.

Discussion: The results of this study showed that the depression and loneliness levels of the elderly living alone or in nursing homes were higher than those of living with their families, and their quality of life and cognitive levels were lower.

Comparison between the laparoscopic Burch and transobturator tape procedures in surgical management of stress urinary incontinence

Abstract

Aim: To compare the results of Laparoscopic Burch and Transobturator Tape (TOT) procedures in patients with stress urinary incontinence.

Material and Methods: In this study which utilized a retrospective cohort study, medical records of patients who underwent TOT (n=33) and Laparoscopic Burch (n=19) procedures at Okmeydanı Prof. Dr. Cemil Tascıoglu City Hospital between 2019 and 2023 were evaluated. Surgical outcomes of the two techniques, including postoperative recurrence rate, intraoperative complications, and efficacy, were comparatively analyzed between the groups.

Results: There were no significant differences were observed with regard to age, gravida, intraoperative complications, efficacy, and postoperative recurrence rate of the patients. The surgery duration was significantly shorter in favor of TOT (p<0.05).

Discussion: Although Laparoscopic Burch operation seems to be an important alternative for those who continue stress incontinence after sling operations, the “long learning curve, operation time and perioperative organ injuries” constitute its most important disadvantages. Our study suggests that TOT is associated with shorter operation time compared to the Laparoscopic Burch. While the short learning curve and operation time are important advantages of TOT procedure, complications such as mesh erosion constitute its disadvantages.

Abstract

Aim: The link between cancer and inflammation is of interest to many researchers. This study aims to determine the relationship between inflammatory parameters (SII, NLR, PLR, and MPV) and diagnostic parameters of 68Ga-PSMA PET/CT (SUVmax and HU). In addition, we aimed to compare these data with PSA values and metastasis patterns in prostate cancer patients.

Material and Methods: Seventy-five prostate cancer patients who were sent to the Nuclear Medicine Department for 68Ga-PSMA PET/CT imaging, who did not undergo a undergo a prostatectomy and radiotherapy, and who had a complete blood count within a few weeks before or after imaging were included in the study. SII, NLR, PLR, and MPV were studied as inflammatory parameters. HU and SUVmax of the areas showing the highest uptake from the tumor areas in the prostate were measured from 68Ga-PSMA PET/CT.

Results: Metastasis was present in 58 (77.3%) patients. Lymph node metastasis was found to be more common in patients with SII≥726 and NLR value ≥3 (p=0.007 and 0.025, respectively). According to median MPV values, patients were divided into two groups: patients with MPV values <9.5 and ≥9.5, and diffuse bone metastasis was found to be higher in patients with MPV values <9.5 (p=0.023). Also, a negative correlation was found between MPV values and PSA (p=0.008,r=-0.310).

Discussion: A statistically significant relationship between high SII and high NLR values and lymph node metastasis, a negative correlation between MPV and PSA, and a relationship between low MPV values and widespread bone metastasis was found. For this reason, it should be kept in mind during clinical follow-up that PSA levels, as well as hemogram data, carry signs of possible metastasis.

The effect of patients’ education level on bowel preparation before a colonoscopy

Abstract

Aim: Colonoscopy is the most important method for diagnosing of colorectal cancers worldwide. One of the most important factors affecting the diagnostic accuracy of colonoscopy is proper bowel preparation. The aim of this study was to determine the effect of patients’ education level on bowel preparation before a colonoscopy.

Material and Methods: Patients who underwent colonoscopies performed by the same general surgeon between January and June 2021 were included in the study. Demographic data, including age, gender, and education level of the patients were recorded. Patients were divided into two groups according to their education level: those with low education level and those with high education level. The adequacy of bowel cleansing of the patients was determined by using the Boston Bowel Preparation Scale (BBPS) 4. Patients with and without adequate bowel preparation were compared in terms of age, gender, educational status, and BBPS score.

Results: A total of 120 patients were included in the study. The mean age of the patients was 50.5 (±11.5) years. 54 (45%) were female and 66 (55%) were male. 32 of the patients had inadequate bowel preparation (26.7%). When classified by education level, 77 (64.2%) were included in group 1 (low education level) and 43 (35.8%) were included in group 2 (high education level). BBPS score was 7.1 (±1.9). The mean age was 49 years (±12.1) in patients with adequate bowel preparation and 54.5 years (±8.3) in those with inadequate bowel preparation (p=0.001). The number and proportion of patients with low education level were 52 (59.1%) among those with adequate bowel preparation and 25 (78.1%) in patients with inadequate bowel preparation (p=0.04).

Discussions: The key factors affecting colon preparation are education level and age. It was found that bowel preparation before a colonoscopy was better in patients with higher education level and younger patients.

Biomechanical comparison of rotational stability of long and short plates in fixation of high tibial osteotomies in the surgical treatment of osteoarthritis

Abstract

Aim: High tibial osteotomy (HTO) is a widely performed surgical procedure for the treatment of medial-sided knee osteoarthritis, especially in active young, middle-aged patients. In this study, we aimed to compare the rotational stabilities of long and short plate fixation of biplanar medial opening wedge osteotomy in artificial tibia.

Material and Methods: Six identical artificial left tibias were assigned to the two separate groups. The first group consisted of the subjects who had undergone medial opening wedge osteotomy fixed with short plates, and the second group with long plates. Twelve artificial seaborne left tibias were used in this study.

Results: The subjects had proper biomechanical properties with ISO-9001 and ISO-14001_en quality standards in Turkey. Maximum torque, yield torque, stiffness torque, and angle values were calculated and compared for both the short and long plate groups. The values for the short plate were as follows: Max Torque:16.16±1.58, Yield Torque: 10.46±1.95, Stiffness: 0.40±.04, Torque: 3.33±76.44, Angle: 63.63±109.33, and Time 28.83±6.96. The values for the long plate were as follows: Max Torque: 19.71±4.17, Yield Torque: 11.95±3.13, Stiffness. 0.45±.08, Torque: 19.66±31.93, Angle: -20.77±54.26, and Time: 29.33±7. There was no statistically significant difference between the groups regarding the rotational stability, which was evaluated by MaxTorque, YieldTorque, stiffness, and angle time parameters.

Discussion: The use of a long plate for the fixation resulted in skin irritation in the distal part of the plate and a bigger incision with a longer time of surgery. We suggest short plates in HTO with biplanar osteotomy as short plates yielded similar biomechanical properties in terms of torsional forces.

Postoperative alopecia: A rare complication following total thyroidectomy and lateral neck dissection

Abstract

Postoperative alopecia is an uncommon complication following total thyroidectomy and lateral neck dissection, typically used to treat thyroid cancer. This case study details a 25-year-old male who, after undergoing these procedures, developed alopecia in the occipital region. The patient had a history of papillary thyroid carcinoma and underwent surgery to address cancerous nodules and lymph nodes. Post-surgery, he experienced serous discharge from the occipital scalp, and by the 20th postoperative day, localized alopecia was noted.
Possible causes of this rare complication include pressure on the scalp during surgery, ischemia of subcutaneous tissues, or the effects of antiseptic agents used in the procedure. The prolonged surgical duration and patient positioning may also contribute. Proper positioning and cautious use of antiseptics are crucial in preventing such issues. While treatment initially involved topical steroids and local care, similar cases often resolve with appropriate management, though there remains a risk of permanent alopecia.
This case underscores the need for vigilance in patient positioning and postoperative care to prevent rare complications. Multidisciplinary collaboration can enhance the management of such conditions, emphasizing the importance of early diagnosis and intervention.

Abstract

Mortality and morbidity in diabetic patients are influenced by a multitude of factors, including genetic susceptibility, environmental influences, uncontrolled medical and surgical histories, and the impact of diabetes on bones and fractures. In type 1 diabetes (T1D), bone mineral density (BMD) tends to be low, elevating the risk of fractures. In type 2 diabetes mellitus (T2DM), osteoblast function is impaired, leading to less mature bone formation, and accelerated bone resorption by osteoclasts. Therefore, it is reasonable to assume that some antidiabetic medications may affect bone density. It is crucial to have updated screening and prevention programs in all countries where diabetes is increasing.
This paper will discuss the impact of diabetes mellitus on bone health to enhance understanding among students and healthcare workers.

Artificial intelligence-assisted triage in the emergency department: Current status and future perspectives

Abstract

This review article examines triage with artificial intelligence in emergency departments and its future role. Currently, the increasing number of patients, limited resources, and challenges experienced in emergency departments make effective triage indispensable. The increasing use of technology in every field is parallel to the increasing use of technology in health services. This paper discusses AI-assisted triage systems, their advantages and challenges. It emphasizes that AI-assisted triage can further improve patient assessment speed, balance resource utilization, and provide consistent triage decisions. However, data privacy, algorithmic biases and healthcare professionals’ adoption of these systems remain significant challenges. In conclusion, AI-assisted triage systems have the potential to improve emergency department efficiency and patient care quality when used in combination with traditional methods.
