Predictive Value of Ischemia Modified Albumin in Determining the Severity of Coronary Artery Disease

Abstract

Aim: Diagnosing myocardial ischemia in patients with chest pain via non- diagnostic electrocardiograms (ECG) and cardiac markers is challenging. The objective of this study is to determine the predictive value of Ischemia Modi- fied Albumin (IMA) levels for coronary artery disease (CAD) severity in Unsta- ble Angina Pectoris (USAP) patients. Material and Method: One hundred and twenty-five patients with chest pain and non-diagnostic ECG findings were included in the study. All patients underwent coronary angiography and CAD severity was evaluated by the Gensini scoring system. Patients were divided into two groups. IMA levels were measured by spectrophotometry. Results: Mean IMA levels were significantly higher in patients than in healthy persons (0.762±0.059; 0.681±0.055 ABSU respectively; P<0.001). In the ROC analysis the cut off value of 0.718 demonstrated CAD with a sensitivity of 76% and specificity of 74%. Positive correlation was found between IMA levels and Gensini score (P < 0.001). Discussion: IMA is a strong predictor of CAD in patients with unstable angina pectoris and is positively correlated with the severity of the disease.

Autologous Conditioned Serum Treatment in the Knee Osteoarthritis

Abstract

Aim: Knee Osteoarthritis (OA) is a major cause of pain and disability in older adults. Of the cytokines identified in osteoarthritic joints, interleukin-1 (IL-1) appears to be of particular importance. The present study is a randomized, prospective, single-institution study to determine the effect of interleukin 1 ß receptor antagonist (IL-1ß Ra) in OA; we made this analysis by injecting sym- metrically involved knees. Material and Method: The present study comprised 33 patients (19 female, 14 male) with 66 knees with a mean age 57.6 (range 41-70 years). Patients were included in the study if they had radiologically- verified bilateral grade 2-3 OA of the knee according to Kellgren-Lawrence classification. The patients were analyzed with Visual Analogue Scale (VAS) for pain, Knee Injury and Osteoarthritis Score (KOOS), and Knee Society Score (KSS) before the first injection was administered (pre-treatment) and again one year after the last injection (post-treatment). Results: Treatment with ACS consistently showed significant decrease in VAS score (p<0.01). The patients reported a significant improvement in KOOS (subscores and total) one year after ACS treatment (p<0.01). The increase in KSS score from base- line to post-treatment one year also was significant (p<0.01). Discussion: Intra-articular injection of ACS (Orthokine) in patients with painful knee OA has an excellent safety profile and results in a strong clinical response. The data show that ACS (Orthokine) represents an effective and well-tolerated alternative to currently predominant treatments of OA.

An Epidermal Cyst Mimicking a Pilonidal Cyst: Case Report

Abstract

Epidermal cysts consist of connective tissues including skin appendages coated by epithelia and the wall which may contain keratin, sebum, or follicle. Intergluteal sulcus is the specific region of localization for pilonidal cysts. Development of epidermal cysts in this region is extremely rare. We present a 17-year-old woman with an epidermal cyst in the intergluteal region that mimicked a pilonidal cyst ab- scess. A 17-year-old woman presented to the emergency service with complaints of pain and swelling in the intergluteal region. A mass excision was performed. In the histopathological examination, the mass was reported an epidermal cyst. The possibility of an epidermal cyst should be kept in mind in the differential diagnosis of benign and malignant diseases of the perianal region.

Squamous Cell Carcinoma of Nasal Septum: Case Report

Abstract

Primary nasal septum tumours are extremely rarely seen among nasal cavity tu- mours and mostly originate from the end section of the caudal septum. Septum tumours are seen with complaints such as recurrent epistaxis, nasal obstruction, discharge, facial oedema or pain. In the examination of a patient who presented with complaints of breathing difficulties, there was seen to be a bleeding lesion of approximately 20 x 15 mm which was crusted over and raised from the mucosa, over the medial crus in the anterior and extending to the alar cartilage. Squamous cell carcinoma was reported as a result of the biopsy. The lesion was excised en bloc and the defect was reconstructed with a sublabial pedicled flap. Although local recurrence rates are high in nasal septum carcinomas, these are tumours which respond well to treatment if determined in the early stages. Treatment of nasal septum malignancies includes surgical treatment, radiotherapy or combined treatment.
